A person can be ready to work on recovery and still feel stuck by everyday problems: missing identification, several appointments, confusing applications or no reliable ride. Case management focuses on connecting needs with resources and following through on a plan. It can help make a complicated situation easier to organize.
Start with the barriers that matter to you
In Kentucky, targeted case management is designed to help eligible people access needed medical, social, educational and other supports. The process includes assessing needs, developing goals and coordinating access to services. Eligibility depends on the applicable program and your circumstances.
An initial conversation should include your priorities. Perhaps the most urgent issue is getting to an appointment; perhaps it is understanding a letter or reconnecting with a provider. You can ask, "What can we work on first, and what will each of us do?"
Turn a large problem into a short action list
For example, a person who needs identification may first need to learn which documents are required, locate those documents and arrange a visit to the appropriate office. A case manager may help identify resources and coordinate these steps within the agreed care plan.
Other goals may involve accessing health care, exploring housing resources or connecting with educational and employment services. Assistance does not guarantee that an outside agency will approve benefits, provide a home or offer a job. Ask what the case manager can help arrange and what decisions belong to another organization.
Understand how the roles fit together
Case management is different from therapy, prescribing and peer support. Your therapist may work with you on emotional or behavioral goals, while a case manager helps coordinate access to services. The roles can support the same overall plan without being interchangeable.
Ask how information is shared, which permissions are needed and who to call about a particular concern. Our BHSO guide can help you ask about the actual services available within a behavioral health organization.
Prepare for a productive meeting
Bring a short list of your priorities, upcoming appointments and questions. Ask which documents are useful and how to provide them securely. Before the meeting ends, agree on the next step, who is responsible and when you will check in.
