Leaving a treatment program can bring relief, pride and uncertainty at the same time. You may be returning home, moving to supportive housing or arranging outpatient care. A transition plan makes the next steps more concrete, especially when different organizations need to coordinate across Eastern Kentucky.
Turn a referral into a confirmed appointment
Before leaving, ask for the next provider's name, address, phone number and appointment date. Check whether the referral has been accepted or whether you still need to call. If a wait is expected, ask the discharge team what support is available during that gap.
Ask how relevant records will be shared with your permission. Keep your own copy of the discharge instructions somewhere accessible. You should be able to explain what happens next without relying on memory after a busy final day.
Keep medication and medical follow-up organized
Review the medication list and follow-up plan with the clinical team. Ask who will handle ongoing prescriptions, how to contact that office and what to do if you cannot obtain a prescribed medicine. Do not stop or change medication without guidance from the prescribing professional.
Write down medical appointments as well as counseling sessions. If you need help coordinating several services, ask whether case management may be appropriate for you.
Make the housing and transportation plan specific
Confirm where you will sleep and how you will get there. If considering sober living, check acceptance, costs, rules and arrival arrangements directly with the home. A referral is not the same as a reserved place.
For each appointment, identify the ride, pickup time and return plan. Read our transportation questions guide if getting to care could be a barrier. Tell the care team early if the proposed plan is not realistic.
Know who to contact when things get difficult
Work with your treatment team on a written plan for warning signs, coping strategies and people you can contact. Ask which number to use during office hours and which instructions to follow outside those hours. Include trusted people only in ways that respect your wishes and privacy.
A difficult day is a reason to reach for support. Review peer support options and arrange follow-up before problems pile up. To ask about continuing care with Ally in Hazard, Jackson or Whitesburg, call 888-920-9004. For a medical emergency, call 911.
